Death certificate or Birth certificates are two opposite certificate issued to the person last time or first time respectively. So the things and matter are different. If someone takes birth at the hospital like me, the hospital might be far from the home place and holding another authority. So hospitals should inform the birth/death but it was not in practice especially in rural, semi-urban or small cities. But who has to face, the parents or the person. So try abolish this.
Death information
It is the liability of docter to issue death certificate mentioning Aadhar No of deaceased.
The death certificate shall be loaded in Aadhar card website.
Within a week Aadhar website issue e death certificate
Birth certificate
Made it mandatory for all hospital to inform all child birth to the Aadhar website.

After some time parents of the baby fulfill all other formality on the website
RESPECTED SIR,
AT PRESENT THERE IS LOT OF CORRUPTION TAKING PLACE JUST TO TAKE BIRTH OF DEATH CERTIFICATE FROM LOCAL MUNICIPALITIES OR CORPORATIONS. SO REQUESTING CENTRAL GOVERNMENT TO MAKE IT MANDATORY FOR HOSPITALS TO FILE DAILY REPORT ONLINE TO MINISTRY OF HEALTH REGARDING BIRTH AND DEATH OF PATIENTS.FROM SAME WEBSITE THEN BY PROVIDING PAN OR ADHAR NUMBER PARENT CAN DOWNLOAD THAT CERTIFICATE. FOR DEATH HAPPENING AT HOME STATEMENT OF LEGAL HEIRS OR LOCAL DOCTOR CAN BE UPLOADED ON SAME WEB.
PM Modi government immediately should announce to public 1 Family, 1 child, like currency demonetization.In present Digital India procedure this is possible.If more than one child,then there will be no birth certificate, no school admission, no addition of name in ration card,no passport, no voting right,no any paper service from government side.Then everything will be alright. The resources will be protected, housing, food, education, medical all will get all. Do this to protect Bharath

Below are my suggestions:
1. There should be a centralized system which provides a single window system to register births and deaths.
2. It should be mandatory for the hospitals and doctors in practice to report any birth/death in the above system.
3. For this all the doctors / hospitals should be mandated registration on the above system
4. We should use technology like block chain to store these public records so that once they're correctly stored they cannot be tampered with.
For getting a death certificate, doctor's certificate is necessary. Many docotors refuse to issue a death certificate. I had a experience recently wherein the doctor who attended refused to issue a death certificate fearing presumed court formalities in case of any dispute. We could get it somehow but many should be facing this problem. There must be provision that no doctor should refuse, but he should not be hauled up in case of any disputes.

Suggestions:
1. Create centralised online system & make hospitals mandatory to upload documents related to death & birth on the same system.
2. Allow access to this system only to govt organizations for privacy security like 'Municilal Corporation'.
3. Create single window online system & allow citizens to apply for birth & death certificate on same system.
4. Citizens should go to respective govt office only to collect the finalized birth/death certificate.
5. 'One Nation One Window'
