How can we maximize health returns through strengthening Regulation of Drugs, Food and Medical Practice?

1. Issues

1.1. Irrational prescription practices for drugs and diagnostics leads to overuse and wastage, as well as drug resistance.

1.2. Aggressive marketing and promotion activities of pharmaceutical companies augment irrational use of drugs.

1.3. There is concurrent jurisdiction of the Centre and States over drug regulation leading to dilution of accountability.

1.4. There are no regulations to control the sale and consumption of unhealthy foods in the population, particularly among children.

1.5. There is a lack of regulation to address the deficit of doctors in rural areas.

1.6. Irrational practices in clinical care continue because of non-adoption of standards of care and limited adoption by States of the Clinical Establishments Act (CEA).

2. Suggestions

2.1. The Standard treatment guidelines should be completed, widely disseminated, mandated and followed to promote rational prescription practices.

2.2. There should be effective enforcement of the guidelines on sale and prescription of drugs, especially antibiotics.

2.3. A mandatory code for identifying and penalizing unethical promotion by pharmaceutical companies is required to be developed.1 This includes legislation requiring drug companies to disclose payments made to doctors for research, lectures, consulting, travel and entertainment that may lead to conflicts of interest.

2.4. Systems of drug regulation need to be reformed since it is under the overlapping jurisdiction of Centre and States.

2.5. Suitable regulation to prevent the sale and consumption of unhealthy foods, particularly among school going children, must be in place.

2.6. There should be an appropriate regulatory mechanism to ensure compulsory rural service by medical graduates.

2.7. The conflict of interest arising due to private practice by Government doctors must be addressed through appropriate regulation.

2.8. The adoption and implementation of the Clinical Establishments Act which includes registration, standards of care, patient rights and grievance redressal mechanisms must be encouraged. Provisions to ensure these regulatory measures may be suitably incorporated into the MoU signed by States with the Centre.

2.9. Prescription audits may be undertaken by professional councils and faculty in medical colleges to ensure clinical decision making complies to evidence based protocols for care, thereby protecting patients from irrational practices.

Dear Modi ji aapse nivedan hai ki jo state goverments ki free medicines yojna ke tahat medicines supply ki ja rahi hain aur unko prescribe karne k liye doctors ko official strict mandate diya gaya hai usme daya kar k medicines ki quality sudharwane ka prabandh karaye ya phir doctors ko free hand de. paisa hote hue patients sub standarf medicine lene k liye majboor hain kyun k govt k order k wajah (darr )se doctors achhi medicine likhne k liye tayaar hi nahi.do somethin 4 this in orisa

Sudhir ji aap ne sahi kaha. Coz unhe b pata chale ki waha kaise elaz hota hai or chakar katate katate . 2 ghante opd me lag jate hai fir doctor 10 bajay aate hai or jab tak doctor se milane ka time aata hai tab tak Lab band ho jati hai fir dusare din wahi chakar ek lab se dusari Lab tak pahuchane me time lagata hai or fir waha apne no. Ka wait j Karte-2 Lab band. Jada problem seinor cittizen or pragnent womens ko hoti hai

महोदय मेरा सुझाव है की समस्त नेता व मंत्री रिजर्व्ड कोटा वाले चिकित्सकों से ही इलाज करवाये विदेश जा कर जनता द्वारा श्रम से उपार्जित धन देश में ही रहने दें

Modi ji, we havve a lot of problems in our country. But, when we consider food is one of our important basic need and it is playing the most important role in creation of a strong country,please do the necessary actions and implement tight control measures all over the country. We are eating poisonous food everyday. No matter if it is from a restaurant or we get it from market to cook ourself. From the ready made food to cultivated food, all are reported by daily news about issues.

Hello Sir, If you see oversea companies are conducting majority of clinical trials in our country and also outsourcing post marketing system too. As we all know very well that in foreign countries the law against pharmacovigilance is very strict on reporting the adverse events with medicines they take. I request you to spread general awareness on pharmacovigilance and its importance. The people of our country should be educated on reporting the adverse events to health care providers.

Very high health and safety standards should be introduced. India has copied quite a few legal system from UK. And one of the best standard in UK is health and safety. One who breaks the standards should be debarred from doing business so that we can avoid cases like Maggi and food adulteration.

Sir u can see this it is not only in our state in each and every state it is a huge problem they are playing with peoples life. A separate wing is needed only for catching these people because it costs most of the life's only a life gone it cant be brought back

Modi ji there should be some regular testing by government on medicines to avoid fake drugs & find expiry medicines same time which help us to ensure all drugs & medicines are as per our Indian law. why don't government starts medicine shop in all rural areas...tribal areas

Main problem of Indian people health is affected by the duplicate or cheap quality medicines in-spite of many drug rackets caught there were no proper measures.I can give simple example that go to general 10 medical shop in different areas and ask him paracetamol u can observe that no one gives same tablets and most of them are not recognized by IPA. If u are able to control this automatically india's health will improve.This can be only possible by either store by govt or strict organizing.
