India has made remarkable achievements in areas like Polio elimination, lowering fertility and disease control. However, our progress in health outcomes has been slower in comparison to other countries with comparable incomes and at similar stages of development. Impressive gains in per capita income should match with increase in life expectancy or health status. We now face a triple burden of disease. Out of pocket expenditures in India is high (70 percent of total health expenditure). This is catastrophic for the poor and pushes an estimated 37 million into poverty every year.

Health is a subject allotted to the State List, under the Seventh Schedule of the Indian Constitution. The Central Government is jointly responsible for items in the Concurrent List.We have one of the most expansive publicly provided networks of health facilities yet issues of regional disparity, access and quality remain. The private sector despite being utilized by the majority of the population also has issues of quality and cost.

Even though the Union Budget allocation for the Ministry of Health and Family Welfare in 2015-16 has remained at the level of revised expenditure in 2014-15, an opportunity lies in encouraging States to spend more on the social sector with greater devolution of untied funds following the recommendations of the Fourteenth Finance Commission.

India is brimming with possibilities. Successful conduct of election, Census survey, projects in space and atomic sciences are some examples. India is termed as the “pharmacy of the global south”, providing affordable, life saving generic medicines to developed and developing countries. In the same way, there is potential for our health system to deliver optimal outcomes to the population.

The Twelfth Plan charts the path towards strengthening health systems so as to reach the long term objective of Universal Health Coverage (UHC). It is our belief that a Health System Strengthening approach is the solution to bridging the gap between our current status and potential performance. The Health Division of the NITI Aayog invites you to an open and informed discussion to elicit ideas for overcoming the enormous challenges in the sector with limited funds at hand and guide future action at all levels, in our system. Your opinion is important and valued.

Detailed note on the current status of health system in India

We invite your responses on these two questions:

1. How can we maximize health returns from available resources?
2. How can we increase investments in health?

This discussion is open for the next two weeks after which we will post a summary of the ideas generated on the forum. We will also follow-up with a more detailed note on the issues in health system and learning from best practices in the country as well as globally for further discussion. Informed briefs on the above mentioned questions will also be made available after the initial two weeks for more a targeted dialogue.

Thinking in terms of mental health, a whole lot of adolescent and youth population is suffering from mental health ailments. A toll-free number with specialists for counselling sessions could be helpful.

Every medical college including private can be given a rural sector to be adopted, where in special facilities can be availed in terms of medical services by people of adopted rural sector in that medical college. Doctors can take up postings including postgraduates in the health setup of that rural sector and refer to medical college if further management is needed.

There is lot of corruption going on in govt hospitals where ranging from group Ds, doctors and everybody are Unitedly corrupted. Supervision is needed in this regard. Help Centre against corruption should be set up in tertiary govt sectors.

Many doctors who work in govt sector also have private clinics and work in private hospitals. As a result, most of the time, doctors are unavailable at the government hospital for good patient care. The ratio of postgraduate to undergraduate seats is about 1:3... Please increase the postgraduate seats in centralised institutions . include rural service as a curriculum in postgraduate course.

NRHM मे सुधार कि जरुरत है JSY ,JSSK ,MAHER जैसी निरर्थक योजनाये बंद करके . नयी व्यापक योजनाये चालू कि जा सक्ती है यह एक आदर्श project है . जिसका फरक जनजीवन पर पडा है

NRHM महाराष्ट्र में अनचाहे पदो का निर्माण कर यहा रिटायर्ड अधिकारियो को रोजगार देणे का केंद्र बना है बेरोजगारी का क्या सरकारी क्षेत्र मे गलत इस्तेमाल हो रहा है बेवजह के पद निर्माण करने ,फिजूल खर्चा करने हो रहा है।अधिकारियो को मोटी तन्खवा,TA ,DA सूख सुविधाये दे कर उन्हें पला पोसा जा रहा है NRHM मे नियमित लोगो को छोड Contractual emply कि meeting लि जाती ही जिसकी वजह से नियमित कर्मचारी पर कोई जिमेद्दरी नही आती उस वजह से cont . emply का महत्व जादा है .व्यापक सुधार आवश्यक है। एनचाहे पदो को कम किया

पर क्या एक समान्य जन के लिये और उसके स्वास्थ् के लिये अच्छे दिन आये क्या । ये काम बडा और गम्भीर अवश्य है पर असम्भव नही है इसको अवश्य ही किया जा सकाता है और इतने अच्छे ढंग से किया जा सकता है कि भारत वर्ष के प्राइवेट अस्पतालो के संचालक भी अपने परिवार जनो का इलाज सरकारी अस्पतालो में करवाने को लाइन लगा के सामने आने को मजबुर होगे क्योकि जो साधन सरकार सुलभ करवा सकती है वो कोई भी प्राइवेट अस्पताल नही करवा सकते ये मेरा विश्वास है ।

यहां तक की आपके सरकार के मन्त्री तक भी प्राइवेट में इलाज करवाना ही बेहतर समझते है । सरकारी अस्पतालो का प्रयोग मात्र कानुनी खानापूरी के लिये और पोस्टमार्टम रिपोर्ट के लिये आवश्यक रह गया है यह एक बहुत ही नंगा सच है और इसे हमारी सरकार और खास कर के प्राधनमन्त्रिह जी आप को इसे बहुत ही गम्भीरता से देखना होगा तब ही इसकी दशा और दिशा दोनो में परिवर्तन सम्भव है अन्यथा हम सब पर भी लोग बडा व्यंग से कटाक्ष करने से नही चुकेगे आप और आपके मोदी जी अच्छे दिनो की चर्चा तो खुब करते थे

आदरणीय प्रधानमन्त्रि जी आपसे विनम्रता पूर्वक कहना है कि हमारे देश की स्वास्थ सेवाओ और खास करके सरकारी स्वास्थ सेवाओं से आम जनता का क्या आपके सरकारी कर्मचारियों तक का विश्वास पूर्ण रूप से उठ चुका है खुद स्वास्थ सेवा से जुडे कर्मचारी भी अपने परिवार या अपने खुद के अस्वस्थ होने पर प्राइवेट इलाज करना जादा वेहतर समझते है ना की सरकारी इलाज सरकारी इलाज तो केवल बहुत गरीब लोग ही मजबुरी मे करवाते है कोई भी सरकार का कर्मचारी छोटे से लेकर बडे अफसर तक यहां तक की आपके सरकार के मन्त्री तक भी
