Problems :
1. We require to handle the birth and death report or document provided by the respective hospital very carefully.
2. In case if this document get lost somewhere then it will be hectic work to regain the same documents from hospital.
3. And as we know, without those document from hospital we can not get birth or death certificate from respective govt organization like Mahanagar palika....

Birth & Death certificate should be made online and the details of the birth and death details should be filled by respective hospitals and then can download the certificate. The certificate should have option to enter the Aadhar no so that we can keep track of the same.

Just register as a boy or girl as soon as born & give time of 3 years for parents to decide the name then issue birth certificate. Also give them lots of option like online,fill out form,open few centre near to private & local hospital, give responsibility to doctor who deliver baby,give Rs 100 as a gift when you register birth etc
For death certificate, advertise in local news paper, community places where more people generally visit that death registration is compulsory for everyone.

Birth registration
--------------------
The current challenges in birth registration is "Naming the baby". People having awareness of birth registration but delay happen for choosing the name. By creating Aadhar id to a new born baby, the initial registration can be done then the parents can register the name within 1 or 2 months. Aadhar registration cells can be open in every hospitals and villages.
Death registration : Appoint an agent in every 100sqmt surronding

centralized birth/death reg. website of govt. with application status feature in which For Birth/death reg. details of parents/Lr's (name, present & permanent addr., aadhar OR voter id OR Passport number (any one from them), mobile numbers of parents/Lr's, proof of birth/death(scanned copy of document)(which may be issued by respective hospital OR Doctor OR Aanganbadi Person OR cremation centre as the case may be uploaded and later verified by concerned sdm and updated to download final cert.

Introduce an on-line registration (birth/death)system linked with Aadhar UID system. It should be initiated by the nearest relative within 24 hours of the occurrence; then endorsed by the hospital/cremation authorities for final approval by the concerned Municipality or Panchayat. A unique birth/death registration number should be generated. This number should override the requirement of paper certificates thus transforming to paperless environment.
